Heparin-Induced Thrombocytopenia Diagnosis and Management

چکیده

Heparin-induced thrombocytopenia (HIT) is an adverse drug reaction characterized by thrombocytopenia and a high risk for venous or arterial thrombosis.1 It is caused by heparin-dependent, plateletactivating antibodies that recognize a “self” protein, platelet factor 4 (PF4), bound to heparin. The resulting platelet activation is associated with increased thrombin generation (Figure).2 Typically, the platelet count fall begins 5 to 10 days after starting heparin, although a rapid platelet count fall can occur in a patient who has antibodies from recent heparin use.3 Remarkably, transience of HIT antibodies permits safe heparin reexposure in selected patients (for example, heart surgery patients) despite a history of HIT.3,4

Heparin induced thrombocytopenia

Abstract Background and Objectives Heparin is still a commonly used anticoagulant in prophylaxis and treatment of thromboembolic events. Heparin-induced thrombocytopenia (HIT) is a life-threating adverse drug reaction of heparin. The diagnosis of HIT is made based on two important criteria, firstly clinical evaluation and secondly laboratory testing. In this comprehensive review, the authors w...

How I treat How I treat heparin-induced thrombocytopenia

Heparin-induced thrombocytopenia is a prothrombotic adverse drug effect induced by platelet-activating antibodies against multimolecular complexes of platelet factor 4 and heparin. Diagnosis rests on a clinical assessment of disease probability and laboratory testing. Management involves immediate discontinuation of heparin and initiation of an alternative anticoagulant. Because of the frequenc...

Heparin-induced thrombocytopenia screening and management in pediatric patients.

The diagnosis and management of heparin-induced thrombocytopenia (HIT) in pediatric patients poses significant challenges. The cardinal findings in HIT, thrombocytopenia and thrombosis with heparin exposure, are seen commonly in critically ill children, but are most often secondary to etiologies other than HIT. However, without prompt diagnosis, discontinuation of heparin, and treatment with an...

The laboratory diagnosis and clinical management of patients with heparin-induced thrombocytopenia: an update.

Heparin-induced thrombocytopenia (HIT) is a serious adverse effect of heparin exposure that can progress to severe thrombosis, amputation, or death. HIT is an immune response in which antibodies cause platelet activation, platelet aggregation, the generation of procoagulant platelet microparticles, and activation of leukocytes and endothelial cells. Early diagnosis based on a comprehensive inte...
